Occasional dropped ventricular pacing in a patient with no underlying rhythm and an Advisa® dual-chamber pacemaker.

چکیده
An 86-year-old white male with complete heart block and no underlying ventricular escape rhythm was implanted with an Advisa MRI dual-chamber pacemaker. Telemetry recordings revealed occasional, isolated P waves without pacing spikes or paced QRS complexes. Decreasing the right ventricle (RV) channel sensitivity to a value of 1.2 mV or higher eliminated completely this phenomenon. The observance of an occasional dropped ventricular paced (VP) event is possible in the Advisa family of devices and occurs during the collection of reference electrogram (EGM). Specifically, turning on/off the EGM amplifiers results in a disturbance of the RV sense amplifier. The intermittent electrical disturbance occurs infrequently, but timed to a predictable interval—a multiple increment of an hour and 30 s. This specific interval increment is associated with the scheduling of the reference EGM collection. Infrequent, occasional dropped VP events are most likely to occur when collection of the reference EGM is synchronized to an atrial sense (AS) event during complete heart block, since cross-chamber blanking following an AS event is not active in the device.

Pacemaker Twiddler's Syndrome
A 76-yr-old man with unipolar ventricular pacemaker, previously implanted in the right ventricular apex, presented with syncopal attack and twitching of the right pectoralis muscle. He finally was diagnosed as having Twiddler's syndrome. The patient had a pulse rate of 36/min and cannon waves in the neck veins. Chest radiography showed a dislodged pacing lead completely wrapped around the pulse...

Abstract: Background and Aim: Since electrocardiography (ECG) is essential for the diagnosis of pediatric asymptomatic Complete Heart Block (CHB) and it is not usually done in the preoperative evaluation of asymptomatic children, physicians might confront with asymptomatic cases of CHB at anesthesia induction. Case Report: In this study, we introduced a 31-month-old boy who faced CHB after anes...
متن کاملRole of permanent pacing to prevent atrial fibrillation: science advisory from the American Heart Association Council on Clinical Cardiology (Subcommittee on Electrocardiography and Arrhythmias) and the Quality of Care and Outcomes Research Interdisciplinary Working Group, in collaboration with the Heart Rhythm Society.
This advisory summarizes the current database on pacing modalities and algorithms used to prevent and terminate atrial fibrillation (AF). On the basis of the evidence indicating that ventricular pacing is associated with a higher incidence of AF in patients with sinus node dysfunction, a patient who has a history of AF and needs a pacemaker for bradycardia should receive a physiological pacemak...
متن کاملCardiac pacing for severe childhood neurally mediated syncope with reflex anoxic seizures.
OBJECTIVE To determine whether permanent cardiac pacing could prevent syncope and seizures in children with frequent severe neurally mediated syncope, and if so whether dual chamber pacing was superior to single chamber ventricular pacing. METHODS Dual chamber pacemakers were implanted into 12 children (eight male, four female) aged 2-14 years (median 2.8 years) with frequent episodes of refl...
